PSA Tests, in general, are not helping men avoid cancer

A controversial (but well reasoned) report suggests men are wasting time and money getting PSA prostate screening for prostate cancer.   The US Preventative Services Task force has authored this huge study, from which the Wall Street Journal notes:

The task force’s draft statement recommends against screening for prostate-specific antigen, or PSA, for men with no symptoms of the disease. It says there is “convincing” evidence that for men 70 years and older, screening has no mortality benefit. For men 50 to 69 years old, the task force found that “reduction in prostate cancer mortality 10 years after screening is small to none.”

From the report itself we find:

“The possibility is very small that death from prostate cancer is less likely in men whose prostate cancer is detected by PSA screening rather than waiting for clinical detection, and the time to any potential benefit is long,”.

Not surprisingly, urologists and many others who benefit in economic ways from PSA screenings have joined some prostate cancer survivors in opposing the  finding.   Barring statistical errors, however, it appears men should forgo this test.    Proponents seem to be relying on the faulty logic that ignores the damage done by false positives or even correct positives that lead to damaging surgery on a patient who might otherwise have been OK.   The data suggests those who get the testing are no less likely to die of prostate cancer than those who do not.   That simple fact suggests the test is a mistake, even though as usual many patients and doctors will ignore the science in favor of their own personal and economic biases.

Lo/Ovral Recall Information from Pfizer

Women using Lo/Ovral birth control should be advised that there has been a voluntary recall of the pills by Pfizer, the maker of Lo / Ovral.    It appears the dosages may not be correct in some of the pills making pregnancy more likely than with correct dosage.

Pfizer Announces Voluntary Nationwide Recall Of Lo/Ovral®-28 And Norgestrel/Ethinyl Estradiol Tablets Due To Possibility Of Inexact Tablet Counts Or Out Of Sequence Tablets

NEW YORK–(BUSINESS WIRE)–Pfizer Inc. announced today that it has voluntarily recalled 14 lots of Lo/Ovral®-28 (norgestrel and ethinyl estradiol)Tablets and 14 lots of Norgestrel and Ethinyl Estradiol Tablets (generic)for customers in the U.S. market. An investigation by Pfizer found that some blister packs may contain an inexact count of inert or active ingredient tablets and that the tablets may be out of sequence. The cause was identified and corrected immediately.

These products are oral contraceptives indicated for the prevention of pregnancy in women who elect to use oral contraceptives as a method of contraception. These tablets were manufactured and packaged by Pfizer Inc., commercialized by Akrimax Rx Products and labeled under the Akrimax Pharmaceuticals brand. This product is distributed to warehouses, clinics and retail pharmacies nationwide.

As a result of this packaging error, the daily regimen for these oral contraceptives may be incorrect and could leave women without adequate contraception, and at risk for unintended pregnancy. These packaging defects do not pose any immediate health risks. However, consumers exposed to affected packaging should begin using a non-hormonal form of contraception immediately. Patients who have the affected product (lot numbers are provided below) should notify their physician and return the product to the pharmacy.

These products are packaged in blister packs containing 21 tablets of active ingredients and seven tablets of inert ingredients. Correct dosing of this product is important in avoiding the associated risks of an unplanned pregnancy.

This recall is being conducted with the knowledge of the U.S. Food and Drug Administration.

Prescription Drug lists from Wikipedia – a good pharmaceutical resource

As you search or information about Prescription Medications note that Wikipedia has several drug lists.  These drug lists may be somewhat outdated but are generally unbiased and always interesting.  Note the links to lists below.

Even older lists are fairly relevant now, although I’m guessing that more statins are even higher on this list.  Obviously this should NOT be seen as a guide as to what to use but it’s helpful to include with doctor consultations some learning about your condition and the potential medications your doctor will recommend.

Prilosec (generic name Omeprazole)

My Omeprazole / Prilosec entry is personal because I’ve been suffering lately from symptoms that appear to be caused by GERD, also known as “Acid Reflux”.  GERD stands for Gastroesophageal reflux disease and may also be called by the terms Peptic esophagitis, Reflux esophagitis, GERD, Heartburn – chronic, Dyspepsia – GERD.

From the National Institutes of Health we learn the following:

When you eat, food passes from the throat to the stomach through the esophagus (also called the food pipe or swallowing tube). Once food is in the stomach, a ring of muscle fibers prevents food from moving backward into the esophagus. These muscle fibers are called the lower esophageal sphincter, or LES.

If this sphincter muscle doesn’t close well, food, liquid, and stomach acid can leak back into the esophagus. This is called reflux or gastroesophageal reflux. Reflux may cause symptoms, or it can even damage the esophagus.

The risk factors for reflux include:

  • Alcohol (possibly)
  • Hiatal hernia (a condition in which part of the stomach moves above the diaphragm, which is the muscle that separates the chest and abdominal cavities)
  • Obesity
  • Pregnancy
  • Scleroderma
  • Smoking

Note that you can now buy Prilosec or the cheaper-but-almost-identical Walmart version called “Omeprazole” (usually located next to the Prilosec OTC in the pharmaceutical section of the Walmart. As a non-prescription medication I think the dosage is lower. You can also have your doctor prescribe higher dosages, but it may be important that you consult with your doctor even if you use the OTC versions since Omeprazole can cause damage to you or your system if used incorrectly (or, I think even if used correctly though it would seem the benefits of prilosec are probably greater than the risks for GERD sufferers).

I’m just starting on generic Omeprazole I bought at Walmart yesterday. In addition to elevating my head when sleeping I’ve been taking generic Pepto bizmol and generic Zantac “ranitadine” for my symptoms and it does not seem to be helping enough – I still get acid and my throat feels very irritated at night. This mostly disappears in the day which is the reason I’m convinced that GERD is my problem.

Pharmaceutical Manufacturers

Here’s a list of most of the Pharmaceutical Manufacturers with links to their websites.  These are in alphabetical order.    As with any prescription drug inquiry, be sure to consult your doctor before, during, and after treatments with any drugs.

There are hundreds of good online resources for drug research. Until we rebuild Prescription Report, to find more about specific drugs, we recommend you begin with high authority health sites like WebMD as well as Government sites like the FDA.     Add WikiPedia entries for many drugs and then examine the information at the drug’s official company website.

The links below are more for follow ups on complaints, recalls, etc. more than to research any drug in particular.
